District 10 Recap - March 23, 2018

POMA District 10 gathered on Friday, March 23 at the Blue Bell Inn. The lecture for the evening was presented by Dr. Aggon, a breast surgeon at Fox Chase (and graduate of PCOM) who gave us an excellent update on breast cancer diagnosis and treatment. The National comprehensive
cancer network provides free yearly updates and can be a helpful source.

Key points from the lecture include:

  • Radiologists are now required to comment on breast density. Breast density is associated with higher risk.
  • USPSTF guidelines suggest biannual exam starting at the age of 40, suggesting that evidence is inconclusive after age 70.
  • NCCN suggests annual mammograms over age 40, with a joint decision as to when to end screening.
  • American Cancer Society suggests: optional screening ages 40-45; recommended screening ages 45-54; biannual screening starting at 55; and stopping screening when life expectancy is less than 10 years.
  • Despite all screenings, cancers are still missed.
  • Molecular breast imaging is essentially a sestamibi scan of the breast that does not require compression and produces fewer false negatives. It is better for masses which are 1 cm or larger, and for very dense breasts.
  • Staging has changed as of last year.
  • Implant associated anaplastic large cell lymphoma is associated with textured implants in 0.3% of 100,000 women. The cancer shows up as a new seroma.
